TARGET AUDIENCE: Behavioral Health, Billing, Operations, Medical Directors/Providers, and Quality Improvement staff

OVERVIEW: Collaborative Care Management (CoCM), originally developed by the University of Washington, is a cost-effective, evidence-based integrated behavioral health care model. The primary care provider, behavioral health care manager, and psychiatric consultant function as a team, using a shared care plan, to meet the needs of patients with mild to moderate behavioral health concerns. This training is intended to help primary care practice team members gain the foundational knowledge and skills for implementing the CoCM model. After completing this training, participants will be able to translate key processes within their practice setting and integrate CoCM into patient visits.
